Camp Rules and Transportation & Field Trip Authorization

Arrival time is 8:30 a.m. - 9:00 a.m. Breakfast starts at 8:30 a.m. and camp activities start at 9:00 a.m.
Pick up time is 5:00 p.m.
If you are unable to be here for pick up at 5:00 p.m., please make arrangements in advance.
Photo identification will be required to pick up your child(ren).
Only authorized parties that you have on file with New Genesis will be allowed to pick up your child(ren).
In order for your child to walk home from camp, we must have a signed consent from the parent / legal guardian.
Children will not be allowed to leave the camp during camp hours. All children must stay until their parent / legal guardian or authorized adult sign them out.
Children must wear name tags at all times.
Guests will not be allowed during camp hours.
Children are allowed to bring their own equipment for sports, however all equipment must be appropriate and labeled.
Medications must be given to the New Genesis office staff and must be in their original containers.
Children are not allowed to keep the medication on their own persons.
Please leave all valuables and money at home. New Genesis will not be liable for valuables or mo ney or any loss thereof.
No cell phones iPods/MP3 players allowed.
Modest clothing comprising of t-shirts, pants/shorts, and tennis/gym shoes is appropriate.
NO heelys/roller shoes
An extra set of clothing (labeled) is recommended.
In order to swim, children must bring an appropriate bathing suit and towel. Goggles, fins, noodles will not be allowed.
Children are expected to be respectful & courteous to other children, staff and volunteers.
No cussing, name calling or racial comments will be tolerated.
Parent must notify New Genesis if the child will be absent.
Parent/legal guardian agrees to complete and submit all required paperwork prior to the beginning of camp.
Parent/legal guardian agrees to work in a positive manner with New Genesis and their staff.
Parent/legal guardian agrees to share the responsibility for their child's behavior during camp.
Parent/legal guardian voluntarily assumes and accepts responsibility of all risks, dangers and injuries resulting from their child's failure to obey safety regulations and directions of the activity leaders.

I, the parent or legal guarding of the child in this application, consent for him.her to participate in field trips sponsored by New Genesis during the 2008 camp season.

I acknowledge that I have thoroughly read and understand the information contained in this release and the possible risks associated with my minor child's participation.

I acknowledge and agree that I will direct my child to act in a responsible manner at all times during the outings and further acknowledge and agree that I will be held responsible for my child's behavior, that my child must respect the property of the facility and its staff, as well as other participants.

Additionally, I acknowledge and agree that my child must observe and comply with the specific rules and conditions of New Genesis and the affiliated organization. I recognize that certain unavoidable hazards and risks are an inherent part of any physical activity. I hereby release, discharge and/or otherwise agree to hold harmless New Genesis Inc., The MerryMaker's Summer Camp, its employees, agents and associated personnel. whether to person or property, other than that which is a result from the sole negligence of New Genesis Inc. and its staff, for any accident or injury sustained by my child.

I certify that my child is in good health, having no physical limitations that would affect his/her participation in this event, and may participate fully.

I further grant permission to New Genesis Inc, The MerryMaker' Summer Camp and its staff to transport my child during off-site events.
